Interview will be conducted by the Civil Status Registry

Requirements:
  • Baptism certificate of child/subject , if baptised
  • Child's Health card
  • Marriage certificate (for mother if married)
  • Parents' record
  • Supporting documents; valid government issued ID of parties (National ID, passport, driver's licence)
  • Payment Fee 
  • Stamps

LATE REGISTRATION WITH BIRTH NOTIFICATION (HOSPITAL PAPER)

Submit:

  • Birth Notification (Hospital Paper)
  • Affidavit
  • Birth certificate of mother
  • Baptism Certificate of child, if baptised
  • Child's Health Card or letter from school first attended
  • One (1) passport size photo of child; signed befor a J.P./Notary Royal only if the child has no form of picture Identification.
  • Valid Government issued ID of mother/Legal Guardian
  • Stamp Duty: Twenty dollars $20.00
  • Copy of death certificate, if mother is deceased
  • Birth ceritificate(s)/date of birth of child/children born before and or after birth to be registered.

The following must be brought along with the above mentioned documents, only if parents are married and the child was born after marriage;

  • Marriage Certificate of Parents
  • Birth certificate of Father
  • Picture Identification of Father

LATE REGISTRATION WITHOUT BIRTH NOTIFICATION (WITHOUT HOSPITAL PAPERS)

  • Submit all other documents under "Late Registration with birth notification"
  • Three (3) statutory declarations by;
  • Child's mother/legal guardian stating the reason(s) for non-registration.
  • Affidvait from two (2) persons present at the birth (e.g. mid-wife, aunt, grandmother.)
  • Affidavit from prominent person in the district where the child was born.
  • Affidavits must be attested to, in the presence of a Notary/Attorney
  • Picture identification (I.D.) for each deponent

Note: All deponents to appear before the Registrar of Civil Status to be interviewed.

  • Stamp Duty: Twenty dollars (4x $20.00)
